Alternativas conservadoras no manejo do Granuloma Central de Células Gigantes dos maxilares: Revisão de literatura

Introdução: O Granuloma Central de Células Gigantes dos maxilares é definido como uma lesão rara intraóssea, com comportamento clínico e radiográfico variável. Sua manifestação agressiva apresenta crescimento expansivo e doloroso, associado à destruição óssea. O tratamento convencional consiste na excisão cirúrgica. Todavia, fatores como idade do paciente, comportamento e extensão da lesão são tidos como determinantes para a escolha da terapia mais apropriada. Objetivo: Discorrer sobre as principais alternativas terapêuticas conservadoras no manejo do Granuloma Central de Células Gigantes dos maxilares. Métodos: Trata-se de uma revisão narrativa de literatura com base na pesquisa de artigos realizada nas plataformas Pubmed e Google Scholar, entre o período de abril a junho de 2020, utilizando o cruzamento de descritores DeCS/MeSH em inglês, a saber: “calcitonin”, “interferon” e “denosumab”, além dos termos livres “central granuloma of giant cells in jaws”, “central giantcell lesion”, “corticosteroids injections” e “intralesional triamcinolone”, através da aplicação de operadores booleanos AND/OR. Resultados: Os corticosteroides representam a classe medicamentosa mais abordada na literatura e com maiores porcentagens de sucesso terapêutico, prosseguido pela calcitonina, denosumabe e Interferon-α. Entretanto, apesar dos resultados promissores, ensaios clínicos precisam ser realizados para a determinação da real eficácia desses agentes. Considerações finais: Todas categorias de tratamento conservador demonstraram ser capazes de promover redução e/ou regressão completa do Granuloma Central de Células Gigantes dos maxilares, tanto de forma isolada quanto combinadas. Todavia, apresentam como desvantagem a possibilidade de desencadear efeitos colaterais, além de possuir poucos relatos na literatura até o presente momento.
